3 experienced Case Co-ordinators are required to join the busy South East Locality Team based in Phoenix House at Vauxhall. Candidates must have a minimum of 1 years experience working within an OLDER PERSONS community team, assesing and reviewing service users care packages, creating care plans, assisting with Direct Payments and managing a large caseload. Candidates will be performance managed. Candidates must be aware of legislation surrounding the Community Care Act, Mental Capacity Act, Safeguarding Adults, and a knowledge of Framework would be advantageous.
Key responsibility areas:1.To assess the needs of clients requiring the services offered by the Council’s Social Services Department and where necessary, in partnership with Health colleagues.
2.To undertake home visits in order to carry out Contact and/or Overview assessments.
3.To identify appropriate resources relating to the client’s needs.
4.To assess for, design and arrange for the delivery of care plans for people assessed using the appropriate assessment tools and guidance.
5.To undertake, where necessary assessments with other Social Services staff and health care professionals, attend hospital discharge meetings and to participate in case conferences and care planning meetings as required.
6.To initiate packages of care (within established financial limits) and provide on-going co-ordination and care planning.
7.To maintain financial awareness in relation to the delivery of care packages with a view to value for money being delivered.
8.To ensure all financial assessments in respect of all care packages in the community are notified to finance officers as appropriate.To forward all relevant documentation to finance officers as necessary, and appropriately advise service users where to receive benefit advice.
9.To assess the need for physical aids, minor adaptations and alterations and provide daily living equipment as a Trusted Assessor.Assess and prescribe specifications for alteration and minor adaptations and liaise with relevant officers in within the Council’s Housing Department or other approved service providers operating within the Integrated Community Equipment Service.
10.To ensure that the Team Manager or delegated Practitioner Manager is fully informed and that guidance and authorisation is obtained for packages that exceed the agreed financial limit.
Education
Good literacy, numeracy and IT skills.
Willingness and ability to obtain/and or enhance qualifications for development in the post
PLUS
NVQ Level 3 in Promoting Independence or Health & Social Care
Key Knowledge
A good understanding of community care activity in relation to the statutory, voluntary or private social care sector.Relevant Experience
Experience of working within a social care setting and some contact with client groups.
