Qualified Social Worker required to manage a caseload of service users requiring assessment of their needs, carry out assessments and investigations and co-ordinating care packages as necessary.
As part of the team and under the general supervision of a senior practitioner or team manager, you will deliver an effective social work service to adults with Physical and Learning disabilities in Colchester Hospital Essex.
Job Purpose:
Under the general supervision of a senior practitioner or team manager, to deliver an effective social work service to a specific group of service users in Essex.
* To hold a caseload of a high level of complexity and provide case management, guidance, support and expert advice to social care staff, students and partners staff as and when required, so as to maintain the highest level of professional service.
*Facilitate service user choice and empowerment through promoting independence as far as possible, by applying professionally acquired knowledge and skills, to promote quality of life through the application of the principles of the personalisation agenda, such as support planning, at the validation / risk management board discussions and Family Group Conferencing.
*To identify appropriate systems of support with service users and carers to enable them to maintain/improve their quality of life to function as independently as possible.
*To liaise with statutory and non-statutory/voluntary agencies ensuring a multi-disciplinary approach to all aspects of service delivery and to facilitate where appropriate the authorisation and purchasing of services, for both short term and long term needs as identified by the service user and carers.
*To refer, liaise, consult and work effectively with other professionals in order to identify and meet service user and carers needs and to jointly develop services e.g. Social Care staff, Equipment Service, Health Care Professionals, District Councils, Housing Teams, Home Improvement Agencies and contractors.
*Record, investigate and respond to Safeguards and risk management boards, complaints and compliments and provide detailed and professional responses in accordance with relevant standards and time frames.
Accountabilities within Hospital team
-To work in close partnership with all Trust staff to enable a safe discharge from, or prevention of, admission to hospital.
*Assessments are limited to need using framework that supports the single assessment process and the supported assessment process which is used to identify the resource allocation.
*Provision of information, support and guidance to enable people to access services within the community.
*Purchasing of Reablement, interim services or reinstatement of services via the Service Placement Team for those people with eligible needs. Accountabilities within Community Assessment TeamsThe key requirement for this team will be to receive all new referrals from Social Care Direct or Hospitals, working very closely with the Reablement Service. Partnership working is key to the success of this team to enable service users to receive personalised services.
*Assessments will be proportionate to need, using the framework that supports the single assessment process and the supported assessment process, which is used to identify the resource allocation.
*Provision of information, support and guidance by being able to access to services within the community for those people who could be supported by these.Accountabilities Long Term Management - Learning and Physical Disability, Long Term Management - Older Adults and Long term Management Team - Older Adults Mental Health
Skills, Knowledge and Experience
*Diploma/ Degree in Social Work, CQSW, CSS or equivalent
*Evidence of working with a complex caseload
*Ensure a high level of data quality for all information recorded, ensuring that the information is timely, accurate and complete.
It is essential that you are able to drive and have a full driving licence or have access to a car and driver.
