To work within a Social Services Community Older Persons Team which organises the department's response to the community care needs of elderly persons.
Carrying out Community Care Assessments. Reviewing and monitoring ongoing service provision. Liasing with external agency's when necessary.
Managing and maintaining a caseload. Writing and updating reports. Maintaining clear, accurate and timely case records.
Working within the current legislation, the organisations policies and core values and departmental policies and procedures.
Experience required of working with adults in a social care setting. Experience of carrying out reviews and assessments on adult service users preferable. Must possess effective written and verbal communication skills with the ability to write and maintain clear and accurate reports.
