London Borough of Richmond upon Thames Adults and Community Services has a vacancy for a social work qualified Team Manager for our team at Kingston Hospital.
The team consists of 1 Senior Care Manager , 5 Care Managers and 1 administrator.
The team works closely with both our community teams in the borough as well as colleagues from PCT's and neighbouring boroughs.
Main Purpose of the job:
To provide a service through partnership among the professional disciplines within the hospital and the borough, in order to deliver services that:
• effectively assess and support the health and social care needs of older people
• promote health, wellbeing, independence and offer choice in type and location of service.
